Membership Application

To apply for membership, fill out the form below and upload your documents. Your information will be reviewed, and we will respond within 2 business days (but usually a lot sooner).

A few things to keep in mind when applying:

Make sure the images of your documents are legible. If we can’t read it, we can’t accept it.
Be sure your documents are current. We cannot accept expired IDs or recommendations.
Make sure to send the front of your ID showing your name and picture.
Make sure to send either your state issued card or the page of your recommendation that shows your name, recommendation expiration date, and doctor’s signature.
